In order to form the U-15 Fukagawa team for the 2017 season, we will conduct selections for new first-year junior high school students (current sixth graders) based on the following requirements.

We will contact you individually about the meeting time at a later date.

Please fill out the application form (download it here), and be sure to obtain the approval seal from the representative or coach of your affiliated team.
②Please pay the participation fee (3,100 yen) at the Loppi machine located inside the nearest Lawson (or Mini Stop).

Please submit the 'FC Tokyo U-15 Selection [Application Form]' at the time of registration on the day.

② Copy of 'U-15 Selection [Application Ticket]'
Please enclose the above ① and ② and send it by mail to arrive by Aug 12 (Fri) 17:00.

3,100 yen(including insurance fee and commemorative item cost)
