In order to form the U-15 Musashi team for the 2017 season, we will conduct selections for new first-year junior high school students (current sixth graders) based on the following requirements.
□Target
New first-year junior high school students (current sixth graders) field players and goalkeepers
□Number of People
Approximately 10 people

Tokyo Gakugei University Affiliated Koganei Junior High School (Artificial Turf)

・1st Selection Sep 11, 2016 (Sun) *Backup date Sep 18 (Sun)
・2nd Selection Oct 2, 2016 (Sun) *Reserve date Oct 10 (Mon)
・3rd Selection *Only those who pass will be informed
・4th Selection *Only those who pass will be informed
We will contact you individually about the meeting time at a later date.
*If the event cannot be held on the day due to inclement weather or other reasons, it will be held on a backup date.

Please apply using the following steps.

Fill in the required information in the application form on the homepage and proceed with the web application.

Please fill out the application form (download it here), and be sure to obtain the approval seal from the representative or coach of your affiliated team.
②Please pay the participation fee (3,100 yen) at the Loppi machine located inside the nearest Lawson (or Mini Stop).

Please submit the 'FC Tokyo U-15 Selection [Application Form]' at the time of registration on the day.

① Application form with the coach's approval stamp
② Copy of 'U-15 Selection [Application Ticket]'
Please enclose items ① and ② and send them by mail to arrive by August 19 (Friday) 17:00.

Please ensure that the web input procedure is completed and the application form is received by the deadline mentioned above.
□Participation Fee
3,100 yen(including insurance fee and commemorative item cost)
*Please note that the application fee you have paid is non-refundable.
