Selections will be held for new 1st year junior high school students (current 6th graders) based on the following criteria in order to form the 2017 U-15 Musashi team.

Field players and goalkeepers in U-5 (current 6th graders in elementary school)

①Application form (click here to download) Please fill in the necessary information and be sure to obtain the consent stamp of your team representative or coach.
② Participation fee (3,100 yen) Please pay at the Loppi (Loppy) installed in your nearby Lawson (or Ministop).

②Copy of "U-15 Selection [Application Ticket]"
Please enclose the above ① and ② and mail them by August 19th (Friday) 17:00 at the latest.
