1. Download the application form from the following link, fill in the necessary information, and make sure to have it stamped with the approval of your team representative or coach.

2. [STEP2] Paste the "U-18 Selection <Application Ticket>" purchased in [STEP2] onto the application form.
Send to the selection department below by April 21st (Friday).
