1. Download the "Exam Acceptance Form" from the following link and make sure to get the signature and approval stamp from the representative of your team (such as the coach).
※If you are not currently affiliated with the team, the representative's signature and consent stamp are not required.

2. Paste the "Participation Application Ticket" purchased in [STEP2] onto the Examination Acceptance Form.
Send to the selection department below by May 8th (Tue).
