2. Have newly contracted "Docomo MAX" or "Docomo Poikatsu MAX" between Tuesday, November 25, 2025 and Saturday, February 28, 2026
* Contracts for "Docomo MAX" and "Docomo Poikatsu MAX" include both new contracts and plan changes from existing Docomo lines to the eligible plans.
3. You have answered the required information (SOCIO number) in the questionnaire form
