In order to organize the 2022 U-15 Fukagawa team, we will be conducting selections for current 6th graders based on the following criteria.

□Target
Current 6th graders (born between April 2, 2009 and April 1, 2010) who are field players or goalkeepers
※We will strive to ensure that players can commute to the practice location (Tokyo Gas Fukagawa Ground) from their homes within 90 minutes as a general rule.

Approximately 10 people

※If you do not have a J.League ID, please obtain a J.League ID on the application form authentication page and then apply.
*Please confirm the "Guidelines for Preventing the Spread of COVID-19" in the application form and only those who have agreed can participate in the selection.
*After completing the registration in the application form, the application completion screen will be displayed.
※After the application period ends, we will use the email address, postal code, address, and phone number registered with your J.League ID as the contact information for this selection. If there is missing information or any changes, please make the necessary corrections before the application period ends.
※Please note that if you remove FC Tokyo from your favorite club settings before the application period ends, we will no longer be able to send you information from FC Tokyo.

After the payment process is completed, you will receive an email from J.League Ticket (jleague-ticket-auto@pia.co.jp) titled "Confirmation of Ticket Application Details".
※Please set your email settings to receive emails from "jleague-ticket-auto@pia.co.jp".
※If you do not receive an email, please log in to the J-League Ticket Official Website and check if the payment process has been completed from the "Purchase History" button in the upper right menu.

④ Purchase "Selection Participation Tickets" at Seven-Eleven stores.
※When issuing tickets, please inform the store register of the "payment slip number (13 digits)" and issue the ticket.
Please inform the staff that you would like to make an "internet purchase" to proceed smoothly.
※A system usage fee of 110 yen and a ticket issuing fee of 220 yen will be charged per ticket, and a settlement fee will be charged depending on the payment method.
※Please note that if you choose to pay by credit card (including d payment, Rakuten Pay, and Yahoo! Wallet), you will not be able to issue tickets at Seven-Eleven until 24 hours after payment for the purpose of preventing fraudulent use.
※If you want to issue a ticket within 24 hours due to the deadline, please select "Payment at Seven-Eleven".
◇If you have forgotten your "Payment Slip Number (13 digits)"

⑤ Download the "Examination Acceptance Form" from the following and be sure to obtain the signature and acceptance stamp from the representative of your team (such as the coach).
※Currently, no signature or consent stamp from the representative is required for those who are not affiliated with the team.

⑥ Attach the "Selection Participation Ticket" issued at Seven-Eleven to the "Exam Acceptance Form" and send it to the following address.

□Confirmation of acceptance
If you have completed the registration of the application form and sent the "Exam Acceptance Form", we will send you the exam information to the email address registered in your J-League ID at the end of the application period (application form deadline: August 6 (Fri)).
※Please note that if you remove FC Tokyo from your favorite club settings before the application period ends, we will no longer be able to send you information from FC Tokyo.

If you do not receive the notification email by August 16th (Monday), please contact the selection department below.
